Registering with the Portuguese national health service, the Servico Nacional de Saude (SNS), provides access to publicly funded healthcare covering GP consultations, specialist referrals, hospital care, maternity services, vaccinations and prescriptions. For residents of the Algarve, SNS registration is a fundamental step.

Registration takes place at the local health centre (Centro de Saude) closest to your home. The Algarve has health centres in all main towns. You need: a valid passport, proof of address, your Portuguese NIF, and evidence of your right to reside in Portugal.

EU citizens not yet formally resident can access the SNS using the European Health Insurance Card (EHIC) or the UK's Global Health Insurance Card (GHIC) for temporary stays. Long-term residents should register formally.

Upon registration, you will be assigned a medico de familia (family doctor) who acts as your primary care physician and gatekeeper to specialists. Appointments are booked through the health centre or the SNS 24 app. Waiting times for routine appointments are typically one to three weeks. Urgent appointments are usually available the same or next day.

Small co-payments (taxas moderadoras) apply to some services: GP consultations around five euros, specialist consultations seven to eight euros, emergency visits 15 to 18 euros. These are waived for pregnant women, children, people with chronic conditions and those on low incomes. Prescription medicines are partially subsidised.

Hospital care is provided by the Centro Hospitalar Universitario do Algarve (CHUA), with hospitals in Faro and Portimao. The Faro hospital is larger and provides a full range of services. Complex cases may be referred to Lisbon.

The quality of SNS care is generally good, though waiting times for specialist consultations and non-urgent surgery can be long. The system is under pressure from a growing population and a shortage of medical professionals.

Many expatriates combine SNS registration with private health insurance, using the public system for routine care and emergencies, and the private system for faster access to specialists. Private hospitals include HPA Group and Hospital da Luz in Lagos. SNS registration is not just a practical necessity but a requirement for various administrative processes in Portugal.

The SNS 24 telephone service, available by calling 808 24 24 24, provides health advice, appointment booking and triage services in Portuguese and, increasingly, in English. The service operates 24 hours a day and can direct callers to the most appropriate level of care, whether that is a pharmacy consultation, a GP appointment or an emergency department visit. Using SNS 24 before attending a hospital emergency department is encouraged by the health authorities and can reduce waiting times.

Pharmacies in the Algarve play a more significant role in primary healthcare than their counterparts in some other countries. Portuguese pharmacists are trained to provide advice and treatment for minor ailments, and many medications that require a prescription in other countries are available over the counter in Portugal. Pharmacies operate on a rota system, with at least one pharmacy in each area open 24 hours on a rotating basis. The address of the nearest open pharmacy is displayed on the door of closed pharmacies.

Mental health services within the SNS are available but often overstretched. Referral to a psychiatrist or psychologist through the public system can involve significant waiting times. Private mental health practitioners, including English-speaking psychologists and counsellors, practise in the main Algarve towns and offer faster access. Several online therapy platforms also serve the expatriate community.
